Supportive Housing Intensive Case Management

God’s Land strives to offer comprehensive wrap around services and meet people where they are at. Our workers have lived experience and have been doing mobile outreach for years. We understand the need does not stop with just housing. Housing stability is achieved by establishing a network of resources and comfort and advocacy navigating those resources.

God’s Land concentrates not just on housing a person but creating a supportive environment through employment assistance or navigation with obtaining benefits assistance with state programs such as Food or Aged, Blind and Disabled Benefits and federal programs like Supplemental Security Income or Social Security Disability Benefits.

Supportive Employment Intensive Case Management

God’s Land helps participants build resumes, write cover letters, practice interviewing skills, search for suitable work and obtain interview and work clothing.

What qualifies a person for services?

Active medicaid

Living with one or more complex health needs and living with one or more risk factors.

The health needs are as follows:

Diagnosed with a mental health disorder, Diagnosed with a substance use disorder, needing assistance with three or more daily activities or disabled and houseless by the coordinated entry definition of houselessness.

The risk factors are as follows:

Chronically houseless or at risk of houselessness for 12 or more months. Within the last 12 months been inpatient in a mental health hospital, hospitalized for a physical need, been to a correctional facility, a detox center, a rehabilitation center, a nursing home or a residential stay facility.
